Prostate

The prostate is a small gland in men that sits just below the bladder. The prostate makes fluid, which forms part of the semen and helps carry sperm.

What is BPH?

THE prostate gland produces the liquid in which sperm are expelled from the penis during ejaculation. As the prostate enlarges, it can press on the urethra, causing problems with urination.

Prostate enlargement is often called benign prostatic hypertrophy or hyperplasia (BPH). It is not cancer and it does not put you at increased risk for prostate cancer. Although an enlarged prostate is not life threatening, it does require long-term management by both the patient and his doctor.

What causes BPH?

In many men, the prostate begins to grow when they are in their 50s and usually continues to grow throughout the rest of their lives. As the prostate grows, it puts pressure on the urethra. As pressure builds, it may lead to bothersome urinary symptoms and future complications.

What are the symptoms of BPH?

Men with enlarged prostates may experience one or more of the following urinary symptoms:

Difficulty in starting to urinate

Difficulty in completely emptying the bladder, possibly resulting in dribbling

A weak urine stream that starts and stops

Urinating more frequently, especially at night (nocturia).
Complete blockage of the urethra can also make it impossible for the man to urinate, leading to a “full” feeling and then to severe pain in the lower abdomen and bladder infections.


How is BPH diagnosed?

A doctor will first take the patient’s medical history and listen to the complaints he is having with urination. The doctor then continues with a physical examination of the prostate. This can be done through a digital rectal examination – that is, the doctor can feel it by inserting a gloved finger into the rectum.

This examination allows the physician to estimate whether the prostate is enlarged, has lumps, or other areas of abnormal texture. Prostate size can also be determined by measuring serum prostate specific antigen (PSA), an enzyme produced by the prostate.

Osteoporosis


Osteoporosis results from depletion of the bone minerals, mainly calcium and phosphates, due to natural process of aging or certain medical conditions (hyperthyroidism, long term steroid treatment). The bones become thin, less dense, brittle and porous leading to easy fractures. Bone mineral loss in normal persons occur starting from about the age of 30 onwards and most severe in women who have early menopause (low estrogen) and older men (low testosterone).


How do you know you have osteoporosis ?

Osteoporosis occurs gradually and may be "silent" until you develop

Red wine and its protective effects on the heart


Since there has been interest shown on wanting to know the efects of red wine on the heart l'll try to explain what's this all about. Let it be forewarned however, that it's a long "lecture" (technical terms are unavoidable) and after reaching the end of it you either understand it, get lost along the way, or becomes tipsy and walk unsteadily. You are advised that you are reading this at your own risk and you should stop immediately anytime you feel dizzy or tipsy.

When one talks about protection on the heart we are referring to trying to decrease the likelihood of coronary events or coronary/myocardial infartc or heart attack in layman's terms.

In the most simplest term, a coronary or myocardial infarct (MI) results from a blocked artery supplying blood and oxygen to the heart muscles. Blockage of the coronary arteries is due to the formation of plugs (aggregation of platelets or blood clots). The causes of plug formation arise from damage to the inner lining of the wall of the artery (as in arteriosclerosis) which is accelerated by high blood pressure and increased “bad cholesterol” (LDL). Arteriosclerosis is basically irreversible fat deposition in the inner lining of artery causing narrowing of the blood vessel. High level of “good cholesterol” HDL confers protection on the blood vessels. HDL and LDL belong to a group of fats also known as lipids.

At present there are numerous epidemiological studies reporting the protective effects of moderate alcohol and wine intake. Effects on lipids and plug formation of different alcoholic beverages have been suggested to play a role. A large database has been provided by the Health Professionals Follow-Up Study in 50 000 male individuals. The relative risk of developing symptomatic coronary heart disease was reduced by one-quarter following wine alcohol intake of 5–30 g/day . These observations are from longitudinal studies whereby a fixed population is followed up for a certain number of years and observed for incidence of coronary events among this population. Others are from epidemiological studies. So there seems to be no controlled studies (how to stop people from drinking wine ah ?) but more data may become available over the years. The mechanisms involved in protection of the heart are numerous. How does alcohol consumption protects the heart ?

A number of mechanisms have been postulated and studied.

1. Effect of wine on blood lipids
Regular alcohol intake increases the blood HDL-fraction. Daily intake of 10–20 g alcohol increases HDL levels by 12% after 4–5 weeks . However, excessive alcohol consumption elevates blood triglycerides (another form of lipids), thereby potentially increasing the risks of MI.

2. Effect on platelet aggregation
Animal studies have provided evidence that the phenolic red wine ingredients trans-resveratrol and quercetin directly inhibit platelet aggregation . In addition, juice from grapes, but not from oranges or grapefruits, can inhibit platelet aggregation. These effects correlate with the concentration of polyphenols in the juices.

3. Effect on inner wall lining of artery
Oxidised LDL has a damaging effect on the wall of arteries enhanced following g a meal enriched by dairy fats. However, when the meal was taken with red Bordeaux wine, there was a significant increase of plasma anti-oxidative capacity. It was speculated that the concomitant intake of red wine reduces oxidation of LDL, which is well recognized to be crucial in the development of atherosclerosis. Finally, the effects of red wine phenolics are more effective than the effect of alpha-tocopherol.

4. Direct effects on blood vessel
Nitric oxide (NO) derived from the wall of artery is an important regulator of elasticity of the blood vessel. Loss of this elasticity (from high blood pressure) is seen in an early stage of arteriosclerosis. Experimental studies show that wine harvested ‘en barrique’ increases the release of NO in coronary vessels. Such effects were not produced by light white or red wines, suggesting that compounds from the oak barrels or phenolic compounds of red wine induce NO-releasing effects. Consistent with these observations, the application of red wine, but not of vodka, produced an increase of coronary blood flow in patients undergoing coronary arteriography. Increased forearm blood flow has been observed following application of red wine, whereas pure alcohol had no effect. Finally, red wine extracts have been reported to increase the activity of promoters of NO release in the walls of artery.

5. Anti-arteriosclerosis properties
The development of arteriosclerosis is promoted by chemicals (PDGF) by stimulating growth and migration of smooth muscle cells in the inner wall lining of artery. ‘En barrique’ harvested wines, like Barolo or Chateauneuf du Pape, were able to inhibit this.

‘The French Paradox’

Coronary deaths are closely correlated with the intake of dairy fat. Death rate from coronary artery disease (CAD) in France (and Switzerland) is only 50% compared with other European countries and the U.S. despite similar intakes of animal fats (a phenomenon termed "French paradox"). The higher wine intake of the French population is believed to account for the lower mortality rate from CAD in France compared with other European countries and the U.S.
Another explanation relates probably to specific procedures performed during wine production. Wine analysis revealed that red wine contains significantly more polyphenols/flavonoids and their bebeficial effects outlined in (2), (3) and (4) above .The long contact time of the originating wine with grape skins and seeds during the mash fermentation leads to an accumulation of flavonoids in red but not white wine. These flavonoids disrupts the processes leading to arteriosclerosis.

Well, it appears that red wine does have a protective effect on your heart. But, hey wait.. do not rush to the pub as yet. An advice you must heed. Too much of wine (don't ask me how many pecks is too much, I'm not a wine drinker) can damage your heart, nerves, liver, brain cells and sexual prowess (you will be reduced to a tikus instead of harimau). So drink in moderation.

WHAT IS? (Medical Terms)

•A diuretic?

What is syphilis?

Syphilis is a sexually transmitted disease caused by bacteria called Treponema pallidum.

Syphilis develops in three stages:-

Stage 1 - known as the primary stage.
Stage 2 - known as the secondary stage.
Stage 3 - known as the latent stage.


How is syphilis passed on and how can I avoid it?

Syphilis is passed on by close sexual contact, including oral sex. Syphilis can also be passed on from a mother to her unborn child.

What are the symptoms?


The symptoms of syphilis are different with every stage, the primary and secondary stages are very infectious.

Primary stage symptoms may take up to 3 months to develop after first becoming infected, they include: -

Painless red sores, at the place where the bacteria entered the body, usually the vagina, vulva, anus, penis or mouth. These sores are called 'chancre' and disappear after 2 to 6 weeks even if they are left untreated.
Secondary stage symptoms develop several weeks or months after the sores appear, if the infection remains untreated.

A non-itchy rash on the palms of the hands and soles of the feet but it may cover the entire body.
Flu-like symptoms - fever, sore throat, aching muscles and loss of appetite.
Patchy hair loss.
The signs of secondary syphilis may come and go over the next 1-2 years. Treatment at any time during these first two stages will cure the infection. It is important to visit your doctor or a G.U.M. clinic if you have any of these symptoms to avoid the progression of syphilis to the latent stage.

Latent stage symptoms develop if syphilis has been left untreated over a period of years. It is at this stage that syphilis can spread through the entire body, affecting the heart, muscles, brain and nervous system. This stage is very rare as syphilis has usually been spotted and treated in the earlier stages. At this stage syphilis is no longer contagious. This stage can last for years or even decades and can result in mental illness, blindness, impotence, heart disease loss of balance and even death.


How is syphilis diagnosed?

The doctor will normally begin by giving you a physical examination. If sores are present, the doctor will take a sample of the fluid and have it analysed under a microscope. A blood test will usually be taken to confirm the diagnosis.


How is syphilis treated?

In the first 2 stages, syphilis is easily treated with antibiotic tablets and/or penicillin injections. You may also be prescribed a lotion for skin sores. Ensure your partner also gets examined and treated if necessary to avoid re-infection. After treatment you should have regular blood tests for 2 years to ensure that syphilis has gone. Once syphilis has been successfully treated it will not come back unless you become re-infected. If treatment is given during the latent stage, syphilis can be cured. However, if the infection has damaged the heart, brain or nervous system before treatment was started, the damage may be irreversible.


What long-term affect will syphilis have?

If a pregnant woman has untreated syphilis she can pass the infection to her baby in the womb. This can lead to miscarriage, stillbirth, or your child becoming infected with syphilis. Children born with syphilis may have symptoms at birth but most develop later. These symptoms include skin sores, rashes, fever, anaemia and various deformities. As infected infants become older they may develop the symptoms of latent stage syphilis, including damage to their bones and brain.

STD-AIDS , some important notes

1. Not all HIV carriers develop AIDS
2. But once AIDS is full-blown, it is considered incurable & fatal
3. Victims usually die within 5 years after full-blown AIDS is diagnosed

Symptoms - HIV infection (early stage)

1. None. HIV has an incubation period of 5 -10 years, a condition that normally last for many years

Garlic Supplements

For centuries since ancient times garlic has been thought to have wide range of properties ranging from fighting infection to warding off vampires ! Ever seen in movies of a garland of garlics worn around the neck of the actor/actress or hung above the door ?

A few studies seem to suggest that garlic may help to lower the blood cholesterol level, lipids (triglycerides) and the formation of atheromatous plaques in the walls of the arteries.

(Plaques are hardened, fatty materials of lipids, cholesterol and calcium that are deposited in the iner lining of arteries. The lead to arteriosclerosis and narrowing of heart arteries)

Recent reviews of scientific literature did do appear to support this. At best, the effects on lowering blood cholesterol is minimal (if any). The other so-called "beneficial effects" on lowering blood pressure and the heart were equivocal and inconclusive at the moment.

The garlic supplement business have capitalised on the earlier perceived "benefits" of garlic. While adding garlic in your diet is not harmful you should be cautious in taking garlic supplements sold on the market.

As in anything, excesses of garlic in the form of supplements can be harmful to your health. Occurrences of fainting and dizziness following "excessive" intake of garlic supplements have been reported.
Like all supplements- minerals, Vitamins, herbs - it is best that you discuss with your family dr as to your individual needs and make an informed decision about increasing your usual intake of these.

How much is a "safe" daily intake of garlic ?


Actually nobody (even FDA) can be sure on this. It is generally recommend eating between half to two average sized cloves of garlic in a day. You should remember that in some individuals, even eating a small amount of garlic a day can lead to bad breath and body ordour because the sulphur (garlics are high in sulphur) is excreted in the breath and sweats

You should be extra cautious if you are already taking other "health or herbal" supplements. When choosing garlic supplements you should choose one that is "coated" so that it is released from the capsule only when it reaches the small intestines and not in the stomach. This is to reduce the bad breath. Its irritating effect on the stomach lining in some people may also lead to belching and of course excessive gas (gentut).

If you are already taking or intend to take garlic supplements it is advisable that you keep in mind the following precautions:

Avoid taking garlic supplements for at least a week if you are going for elective operation. It may interfere with the blood clotting mechanism.

Do you need garlic supplement when you intend to or are currently nursing (menyusu) your baby?

Are you currently pregnant ? What is the effect of excessive garlic on the unborn baby ?

Can excess garlic affect my growing child ?

You have an underlying medical condition involving beeding tendencies or clotting of the blood.

You have been prescribed a blood thining medication (aspirin, warfarin,etc) for your heart condition.

A previous reaction to garlic like urticaria, skin rash, itchiness, swelling of the lips and/or tongue, difficulty breathing, stomach cramps. All these indicate a serious allergy to garlic consumption.

"What supplements to be taken daily" reference, please read.

Vitamin C, also known as ascorbic acid, is a water-soluble vitamin. Like most water-soluble vitamins, excess intake of vitamin C is excreted and not stored in the body.
